In-Office Services:

For Jeuveau treatments, a cosmetic consultation with a licensed provider may be required, typically valued at $150, included at no extra cost for all in-office pre-purchased services. A credit card on file is mandatory for booking any procedure. If deemed unsuitable for the treatment, a full refund will be issued. Pre-payments for services are credited to your account, valid for 1 year from the payment date. Special package rates are exclusive and cannot be combined with other promotions, yet may qualify for Evolus rewards. Should service packages be partially used, refunds for unused services will be calculated at the single treatment list price, with any difference refunded. Offers are subject to change and can be revoked without notice. Completion of patient paperwork is required for all treatments. Offers valid while supplies last, void where prohibited.
