surgical excision types

At Revelus Dermatology, a range of Surgical Excision techniques are available to suit various diagnostic needs. The selection includes shave, punch, elliptical and wide local excisions, each offering distinct benefits for skin lesion removal.

types of surgical excisions explained:

  • Shave Excision: A small blade is used to remove a lesion shave, ideal for smaller, superficial growths.
  • Punch Excision: A small circular tool is used to remove a deeper, well circumscribed skin growth
  • Elliptical Excision: Removal of an area by using an elliptical-shaped incision, allowing for better wound closure and minimized skin tension
  • Wide Local Excision: Used for cancerous growths, this technique involves removing both the tumor and a surrounding margin of healthy tissue to ensure complete removal.

what to expect*

preparation

Preparation for a Surgical Excision is quite simple. You will be scheduled for a dedicated surgical time and asked to arrive early for numbing.

The area to be excised will be cleansed and marked. Photographs will be taken.

procedure

The Surgical Excision procedure itself is straightforward. The area will first be numbed with a local anesthetic agent. Then, the lesion to be excised will be taken by either a shave, punch, elliptical or wide local excision method. Sutures are not necessary for shave procedures but may be utilized for deeper excisions. The procedure itself lasts about 30 minutes on average

aftercare

Proper aftercare is vital for optimized wound healing. Detailed instructions will be provided on how to clean and protect the excision site. It is usually recommended to keep the site clean and dry for 24 hours after the procedure. Vaseline or Aquaphor is to be applied in the following days. Instructions will also be provided on how to monitor for signs of infection. Any mobility or exercise limitations will also be discussed. Not following proper aftercare instructions increases the risk of scarring and other wound related complications.

If needed, a suture removal appointment will be scheduled with a nurse in 1-2 weeks.

resuts + next steps

The results of a Surgical Excision are typically returned within 1-2 weeks but may take longer. The results/findings will be communicated to you and explained. If needed, additional procedures may be scheduled, and any associated treatments based on the findings will be recommended.

common surgical excision questions

FAQ
what is a surgical excision?

A Surgical Excision is a medical procedure conducted by a trained dermatology provider where a specific lesion and area of healthy tissue surrounding it is removed. After removal, the area is sent for pathologic examination under a microscope. This analysis aids in confirming a diagnosis as well as accurately ensuring that a lesion was removed in its entirety.

does a surgical excision hurt?

During a Surgical Excision, the area to be removed is first numbed using a local anesthetic. There is a slight prick and burning sensation associated with the local anesthesia, but this resolves quickly, and the excision is then able to be performed with little to no pain. It is normal for minor pain, tenderness and swelling to occur for several days after a Surgical Excision.

will a surgical excision leave a scar?

The potential for scarring depends on the type of Surgical Excision performed and the individual’s skin type and healing response. While ALL Surgical Excisions will leave a scar, our providers employ expert techniques to minimize this risk and provide guidance on scar care post-procedure. It is imperative to follow all post-procedure instructions, including mobility and exercise limitations, in order to ensure optimal healing.

what are the different types of surgical excisions?

There are four main types of Surgical Excisions– shave, punch, elliptical and wide local.  The chosen excision type is contingent on the site and also the specific skin condition being removed.

  • Wide Local Excision: Used for cancerous growths, this technique involves removing both the tumor and a surrounding margin of healthy tissue to ensure complete removal.
will i need stitches/sutures?

Stitch/suture requirements depend on the excision type. Shave excisions usually don’t require sutures. Punch, excisional, and wide local excisions usually require stitches as a result of the depth and size of the lesion taken.

how long does it take to get the results of a surgical excision?

Results from a Surgical Excision are generally available within one to two weeks. The specimen is sent to a laboratory where it is thoroughly analyzed under a microscope by a pathologist. We understand the waiting can be stressful and we strive to deliver the results as quickly as possible.

It is always best practice to contact the office for your results after 2 weeks if you have not yet heard back.

can a surgical excision diagnose skin cancer?

Yes, a Surgical Excision is a highly reliable method for diagnosing skin cancer. However, excisions are more often used to treat certain skin cancers. By examining a skin sample under a microscope, pathologists can identify cancerous cells and determine the type and stage of skin cancer, if present. Furthermore, pathologists can verify healthy margins to ensure a skin cancer has been fully removed.

what is the cost of a surgical excision?

The cost of a Surgical Excision varies depending on several factors including the type of excision, the complexity of the procedure, and the individual’s insurance coverage. It’s best to consult with your insurance provider for the specifics of coverage. At Revelus Dermatology, we strive to provide high-quality care and transparency about estimated procedural costs.
