Thank you for choosing Revelus Dermatology for your cosmetic care. Please review the following terms and conditions before purchasing a cosmetic special or prepaid service.

Promotional Offers + Eligibility

• Cosmetic specials are available for a limited time and may have additional offer-specific terms.
• Offers are available to legal U.S. residents age 18 or older.
• Photo identification may be required at the time of treatment.
• Packages may be purchased together but are billed, tracked, and scheduled separately.
• Purchase limits may apply. Duplicate orders may be canceled and may incur a 10% processing fee.
• Offers cannot be combined with other Revelus promotions or discounts unless otherwise stated.

Pre-Purchased Services

• Eligible services may be purchased online and are maintained as credits on your Revelus account.
• Pre-purchased services must be redeemed in office.
• Promotional pricing is secured at the time of purchase.
• Service credits expire one year from the purchase date unless otherwise stated.

Consultations + Treatment Eligibility

• Injectable and device-based treatments require a consultation or provider review before treatment.
• Facial services may not require a separate consultation, although provider review or medical clearance may be required when appropriate.
• All treatments are subject to clinical suitability and provider approval.
• If you are not medically eligible for a purchased treatment, the purchase amount may be applied toward another eligible service with practice approval.
• Some treatments may require Medical Director approval.

Clinical Evaluations

• If treatment has not started, a clinical evaluation is generally valid for 90 days. A new consultation or updated quote may be required after that time.
• Once treatment begins, evaluations are generally valid for 180 days.
• Gaps longer than six months may require a new consultation or updated treatment plan.
• Each provider may require a separate consultation or evaluation before performing treatment.

Treatment + Scheduling

• Required intake forms must be completed before treatment.
• Treatments should be completed within the recommended timeline to support safety, consistency, and treatment planning.
• Appointments are subject to provider scheduling and availability.
• Treatments are typically scheduled with the consulting provider unless otherwise noted.
• Full payment is required at the time of purchase to secure promotional pricing.
• A credit card on file may be required to schedule an in-office appointment.

Cancellations, Late Arrivals + No-Shows

• Cancellations require 48 business hours’ notice. For example, a Monday appointment must be canceled by the previous Thursday.
• Late arrivals may result in shortened treatment time or may be considered a missed appointment.
• Missed appointments, late cancellations, or no-shows may result in forfeiture of service value.
• Two consecutive missed appointments may result in forfeiture of the remaining package or service credit.
• Opting out of appointment reminders does not change these policies.

Refunds, Credits + Transfers

• Packages are non-refundable once partially used.
• Unused services may be transferred to another eligible service with practice approval.
• Refunds for unused services may be subject to a 10% processing fee.
• Refunds and credit transfers may be recalculated using standard single-treatment pricing.
• If Revelus Dermatology is unable to provide the purchased service within 60 days due to an operational issue, the processing fee will be waived.

Safety + Results

• Individual results may vary.
• Please inform your provider of any changes to your health, medications, pregnancy status, allergies, or medical history.
• Please follow all pre-treatment and post-treatment instructions provided by your care team.
• Revelus Dermatology may decline or modify treatment based on clinical suitability, medical history, or provider judgment.

Manufacturer Rewards + Financing

• Select treatments may qualify for manufacturer rewards or third-party financing.
• Eligibility, availability, promotional dates, and program terms are determined by the participating manufacturer or financing provider.
• Manufacturer rewards and financing approvals are not guaranteed by Revelus Dermatology.
