Revelus Dermatology Botox Package Promotion – Terms & Conditions



Thank you for choosing Revelus Dermatology. Please review the following terms related to Botox promotional packages.



This promotion applies exclusively to Botox cosmetic packages purchased during the promotional period.



Offers are available to U.S. residents age 18 or older. Valid identification may be required.



Limit one promotional Botox package per patient. Duplicate purchases may be canceled and are subject to a 10% processing fee.



Botox requires an in-person medical consultation prior to treatment. Purchase of a package does not guarantee medical eligibility.



If a patient is deemed not medically appropriate for Botox, the purchase may be applied as a credit toward another eligible cosmetic service.



Pre-purchased Botox units are stored as a credit on the patient’s account and may be used over multiple visits.



All Botox package credits expire one year from the date of purchase.



Payment in full is required at the time of purchase to secure promotional pricing. A valid credit card must be kept on file to schedule treatment.

Appointment cancellations require at least 48 business hours’ notice. Missed appointments are subject to a charge of 50% of the scheduled service value or $200, whichever is greater.

Results vary by individual and are not guaranteed. All required intake forms and consent documents must be completed prior to treatment.

Unused Botox units may be transferred to another cosmetic service with provider approval. Refunds for unused units are subject to a 10% processing fee and will be recalculated at standard per-unit pricing.

If services cannot be provided within 60 days due to provider availability, the processing fee will be waived.
