New patients complete the New Patient Packet, which includes demographics, medical history, our financial policy, and our office policies. Patients under 18 also need a Minor Consent form signed by a parent or legal guardian.

how do i prepare for a dermatology appointment?
To prepare for your appointment, please avoid wearing makeup or applying lotions or creams on the day of the visit to allow for a thorough examination. Bring a list of any medications you are currently taking, including over-the-counter products and supplements. If you have a history of skin problems, consider bringing any relevant medical records or previous test results.

No. Medical and cosmetic patients complete the same New Patient Packet. If a treatment needs an additional consent, our team will provide it at your visit.

where can I find your cancellation policy?
Our cancellation policy is part of the Financial Policy in the New Patient Packet. Please cancel or reschedule at least 24 business hours before your appointment. A missed appointment, or a change made with less notice, is subject to a fee for any reason: $50 for consultations and basic treatments, or $200 for procedures that take 20 minutes or more.
